I haven't received a single payment since you started forcing users to send the W-8 BEN form. I have over $200 sleeping in my account and i want to get it.

I tryed filling the form and faxing it to you but your line is always busy. It says on the website that i can also email it to you, but how am i supposed to sign the form if i send it through email after ?

I'm not even an US citizen (i live in Canada) so i don't understand why i have to fill it.

Also i don't understand how you will be able to check if you received my form since there is no place where i can write my Zazzle account name on the form. How will you associate the form to my Zazzle account ?

I also don't understand the Part 3 of the form. It says i need to provide some sort of statement "that identifies those notional principal contracts from which the income is not effectively connected with the conduct of a trade or business in the US"
What is this statement that I am supposed to provide ?

This is very unclear and confusing. I moved to a new company a couple of months ago because it was too complicated. But i would like to get back on Zazzle if i can get this form done.

Note: I sent e-mails to Zazzle Support about my problem but they never got back to me.

Print it, Sign it, Scan it. I scanned my to pdf files.

Then send as an attachment via email with your user name and store name and 'w8-ben' in the subject line and body of the email.

That should do it, it's not neccesary to tick Part 3.

When it's processed you will see 'We have received and processed your form. Thank you!' on your payment settings page.

Yeah I was confused to tears till I found that thread, thanks very much to the zazzler who uploaded an example copy showing what to put in all the relevant parts.

I've just emailed mine, hopefully it'll be dealt with soon.
